Retained Placenta in Cows: The First 24 Hours

Short answer: If the afterbirth has not been expelled within 12 hours of calving, the placenta is retained and you should call your veterinarian rather than wait it out. Do not pull on the hanging membranes — that is the single most damaging thing a farmer can do. Most retained membranes will eventually detach on their own, but the real cost is not the membrane; it is the uterine infection, delayed involution and lost fertility that follow, and that is what the first 24 hours are really about.

What counts as retained placenta

In a normal calving the foetal membranes come away within about three to eight hours. The accepted veterinary definition of retained foetal membranes (RFM) is failure to expel them within 12 hours of calving. Reported incidence in dairy cattle worldwide sits at roughly 5–15%, so in a herd of fifty calvings a year you should expect to see it several times.

What you will see is a rope of membrane hanging from the vulva, sometimes for days, usually with a foul smell developing by day two or three. Sometimes nothing hangs out at all and the membranes are retained entirely inside — the first sign is then a stinking discharge, a dull animal or a cow that goes off feed around day three.

Why it happens

  • Difficult or assisted calving, twins, or a premature calf — anything that disturbs the normal detachment process.
  • Milk fever and low blood calcium — calcium drives uterine muscle contraction, so a hypocalcaemic cow cannot contract properly. This is one of the most common and most preventable causes.
  • Selenium, vitamin E and vitamin A deficiency, and poor trace mineral status generally.
  • Over-fat cows at calving, and cows in negative energy balance with a suppressed immune system.
  • Induced or early calving, heat stress, and infections such as brucellosis that cause abortion or premature birth.

The first 24 hours: what to do

Hours 0–6 — watch and let her work

Let the calf suckle. Suckling triggers oxytocin release, which drives uterine contraction and helps expel the membranes — it is the most useful free intervention you have. Keep the animal calm, on clean dry bedding, with clean drinking water and palatable feed within reach. Note the exact time of calving; you will need it.

Hours 6–12 — prepare

If nothing has come away by six hours, tell your veterinarian so they can plan. Keep the area clean. Do not start home remedies.

Hour 12 onward — call the vet

Past 12 hours it is a retained placenta and needs professional assessment. Your veterinarian will decide, based on the animal's temperature, appetite and general condition, whether this is a simple retention that can be managed conservatively or whether systemic infection is developing and needs treatment. That judgement matters, because a bright cow eating normally with retained membranes is managed very differently from one with fever and toxic metritis.

What never to do

This section matters more than the rest of the article.

  • Never pull on the membranes. Manual removal and traction tear the caruncles in the uterine wall, cause bleeding, drive bacteria deep into the tissue and can leave fragments behind. Veterinary opinion has moved decisively against routine manual removal precisely because it worsens outcomes. Tying a weight to the hanging membrane to "help it out" does the same damage more slowly.
  • Do not put anything into the uterus — no oils, no home preparations, no boluses — unless your veterinarian has specifically instructed it. The uterus after calving is a wound; contaminating it converts a manageable problem into metritis.
  • Do not cut the hanging portion short. It looks tidier and it is genuinely worse, because the remaining piece retracts inside where it cannot drain.
  • Do not give antibiotics on your own judgement. Not every retained placenta needs them, and unnecessary use costs you milk withdrawal and drives resistance.

Why it matters beyond the membrane

Retained placenta is a gateway problem. The retained tissue is dead material sitting in a warm contaminated space, and it sharply raises the risk of metritis and then endometritis — chronic uterine inflammation that quietly wrecks conception rates. The knock-on effects are a longer interval from calving to first service, more services per conception, an extended calving interval and, in many cases, an animal that eventually gets culled for infertility rather than for the original problem.

This is why post-calving uterine recovery deserves attention even when the animal looks fine. Involution — the uterus shrinking back and clearing itself — should be well advanced within three to four weeks. Lochial discharge should be reddish-brown early on, gradually reducing and clearing. A discharge that stays thick, off-white or foul-smelling past two weeks is not normal and needs examination.

Prevention: the part you control

  • Keep the calving area clean and dry, and give the animal room to calve without interference.
  • Intervene at calving only when genuinely needed, and do it hygienically. Rough or premature assistance causes retentions.

Frequently asked questions

How long can a cow safely have a retained placenta?

Most retained membranes detach on their own within two to eleven days, but "detaches eventually" is not the same as "no harm done". The risk of metritis rises the longer it stays, so the decision point is 12 hours — that is when your veterinarian should be involved, not when you should start worrying.

Should I pull the placenta out if it is hanging?

No. Traction tears the uterine attachment sites, causes bleeding and pushes infection into the tissue, and it frequently leaves fragments behind that cause worse infection than the original retention. Leave it alone and let your veterinarian assess.

Will a retained placenta affect the next pregnancy?

It can, mainly through the uterine infection that often follows. Cows that develop metritis or endometritis after RFM take longer to conceive and need more services. Prompt veterinary management and proper post-calving uterine recovery are what protect the next breeding.
