Short answer: The transition period — roughly three weeks before to three weeks after calving — is the six weeks in which a dairy animal's entire lactation is decided. Almost every expensive problem in a dairy herd, from milk fever and ketosis to retained placenta, metritis and repeat breeding, is either created or prevented in this window. Managing it well is not about treating disease faster; it is about keeping dry matter intake up, calcium status right and the uterus recovering cleanly, so that the diseases never start.
At calving, a dairy animal goes from feeding a foetus to producing milk almost overnight, and her energy requirement roughly doubles within days. Dry matter intake does not keep up. The result is negative energy balance: she mobilises body fat, the liver is loaded with fatty acids, and her immune function is measurably suppressed for several weeks around calving.
Two things happen at once, and they feed each other. Metabolically, she is short of glucose and calcium. Immunologically, she is least able to defend a uterus that has just been through parturition and is inevitably contaminated with bacteria. The classic transition diseases are what happens when those two curves cross.
Every one of these steps is cheaper to prevent than to treat, and most farms spend their money at the wrong end of that list.
Transition failures cost in four ways that farmers routinely underestimate: direct treatment cost, which is the smallest of the four; lost milk in the current lactation, which is large and often never fully recovered; extended days open, where every extra day beyond the target calving interval carries a real daily cost; and involuntary culling of animals that leave the herd for infertility rather than for age or yield. An animal that develops metritis after a retained placenta and then takes three extra services to conceive has cost far more in lost days than in medicine.
Get animals to calving in moderate condition, not fat. Over-conditioned cows eat less around calving and mobilise more fat, which drives both ketosis and immune suppression. Intake in the last two weeks is the single most predictive number you can watch; a measurable drop before calving is a warning.
Practically: avoid overcrowding at the feed face, keep close-up animals in a stable group rather than shuffling them, provide comfortable lying space, and keep feed palatable and available. Calcium management in the close-up period is the highest-leverage intervention available. The aim is to have the calcium regulation system primed and responsive at calving rather than sitting idle. Discuss the right approach for your ration with your veterinarian or nutritionist — anionic salts, controlled dietary calcium and oral calcium at calving all have a place depending on the system.
Trace mineral and vitamin status matters alongside it: selenium, vitamin E, vitamin A, copper and zinc all feed into immune competence and placental detachment. Give the animal space and a clean, dry, non-slip surface. Intervene only when genuinely indicated and do it hygienically — unnecessary or rough assistance is a direct cause of retained membranes and uterine trauma. Let the calf suckle promptly; the oxytocin response supports uterine contraction, and the calf needs colostrum within the first hours regardless.
The highest-value routine on any dairy is a daily fresh-cow check for the first two weeks. Look at four things: rectal temperature, appetite and rumen fill, the character of the lochial discharge, and milk yield trend. A cow that goes off feed on day three is telling you something before she looks sick.
Normal lochial discharge is reddish-brown initially, becoming lighter and reducing over two to three weeks. A discharge that remains thick, off-white, watery-red past two weeks, or that smells foul at any stage, warrants examination. Uterine involution should be well advanced by three to four weeks; palpable failure to involute beyond that predicts poor fertility.

An open-label multi-centre field study of Uterotone 3-in-1 in 51 dairy cows and buffaloes across seven Indian states, published in the International Journal of Veterinary Sciences and Animal Husbandry (2026), reported timely placental expulsion and normal lochial discharge in all treated animals, a mean early-lactation milk yield increase of 541 ml/day among the 40 animals with usable yield data, and no adverse effects. Conventionally three weeks before to three weeks after calving — about 21 days either side. The tightest risk window is the first 14 days after calving, which is when most clinical disease appears and when fresh-cow monitoring pays for itself.
For most Indian herds, calcium management in the close-up period combined with protecting dry matter intake. Subclinical hypocalcaemia is common, largely invisible without testing, and sits upstream of retained placenta, displaced abomasum, metritis and poor intake. Fixing it improves several outcomes at once.
The principles are the same — intake, calcium, hygiene, uterine recovery — but buffaloes are more heat-sensitive, often show less obvious clinical signs, and silent or weak heat expression afterwards makes fertility monitoring harder. Practically that means more attention to cooling and wallowing access in the dry period, and closer observation for heat after calving.
